Tried the trusted device on the Samsung S6. For this particular phone, it unlocks immediately after detecting the NFC ring (without swiping).

Samsung S6
Just to report that the Alpha ring works quite well with the Samsung S6, even through the official Clear Cover. Not really 100% success rate yet, probably there should be another sweet spot somewhere.
Not much luck with the S View Cover for S6 - The back is slightly thicker than the Clear Cover. Not too sure about the Clear View Cover.
Unlocking the Samsung S6 via trusted device is quite painless! :)

RE: Unlock app issues
For your second issue, I remember that you can choose to unlock the phone automatically when the call comes in, and lock the phone again when the call ends. I'll need to reinstall the app on my phone before I can guide you on that.
Alternatively, if your phone is on Android 5.0 and above, you may want to try using the Smart Lock feature. Before setting it up, remember to disable/uninstall the unlock app first. (Though at the moment, there seems to be some issue with the Smart Lock stops responding to NFC tag after some time: https://forum.nfcring.com/topic/534/android-lollipop-trusted-device-nfc-ring-perfect)

RE: S6 native android unlock doesn't require any swipe..
As far as I know, it applies to Samsung S6 (and likely the edge) as well. Had noted this quite some time back: https://forum.nfcring.com/topic/534/android-lollipop-trusted-device-nfc-ring-perfect/20

RE: Samsung Galaxy S4
After using for three more weeks trying out different solutions, I've decided to request for a change to alpha ring instead. Even though the classic ring works well, I can't live without the front cover. Too scary to use a phone without a protective cover, especially for me with butterfingers. :)

RE: Galaxy S4 S-View cover
I can put one up once I reach home tonight, though I never succeed in finding the perfect sweet spot with the S-View Cover on.
I will also put up the S6 sweet spot while I am on it.

RE: Unlock app issues
Unlock Settings > Manage Call Behavior
Change the following:
- Incoming Phone Call Behavior > Disable on phone ring
- Disconnected Phone Call Behavior > Re-lock the device right after the call is disconnected
- Answered Phone Call Behavior > Do not re-lock the device (If I recall correctly, this will allow you to still access the "end call" button)

RE: native nfc lock on samsung s4 lollipop
Not isolated to Samsung only. The most important step is to enable the Smart Lock in Trust Agents. Otherwise the Smart Lock feature will be missing.

RE: Galaxy S4 S-View cover
There is another sweet spot at the opposite end, for those wearing the NFC ring on their left hand.
Interesting tibits: Without the cover on (i.e. exposing the battery), you can use the NFC ring at the bottom and top of the battery as well. Though nobody will use the phone with the battery expose.

Samsung S6 with clear cover
So far the only sweet spot I can find for Samsung S6, with the clear cover. Able to unlock with one hand most of the time (with exception of a few locations, especially that one spot at the undergound subway... * grumble *).
In case someone is wondering about the clear view cover - Nope, the S6 can't read the NFC ring through that cover. It is slightly thicker than the clear cover.
